Modern regulations for retention of medical records transferred patient files from the physical shelves of the physician’s office to an electronic format stored virtually via third party organizations beyond the physician’s office. As such, CND has no physical patient records.

 

Records are maintained for seven years.  If you were treated within the past seven years, you have the right to acquire copies of your records. From whom should you request your records?

 

  • If you were treated by any of our physicians at John Peter Smith Hospital, you should contact JPS Hospital District medical records department and/or ACCLAIM Physician Group.  Phone: 817-921-3431, ask for Medical Records.

 

  • If you were treated at the Center for Neurological Disorders by Dr. Cravens, you should contact the third-party vendor who maintained storage of CND records: Storm Prime and/or Intergy. Phone: 972-573-4925.

 

  • If you were treated at the Center for Neurological Disorders by Dr. Kesterson, you should contact him at: 1450 8th Avenue, Fort Worth, Texas 76104 or 1500 S.
